Step 1 — Submit adoption application

To begin the adoption process, you and your spouse (if married) must fill out and submit a comprehensive application form. This form provides AMI with the background information needed to facilitate an adoption, including:

General contact and employment information
Financial status, including full disclosure of assets (income, savings, property values) and liabilities (mortgage, rent, loans, etc.)
Health status and insurance coverage (medical, life, and disability)
Current children (both biological and adopted)
Educational background
Marital status
Criminal history
Previous adoption experience
Child preferences (age, gender, country, etc.)
Personal references

Along with the form, you must also submit a family photo and a $175 nonrefundable application processing fee.

AMI realizes that the information contained in the application is highly confidential, and we respect the privacy of our applicants. We will only release this personal information to the organizations and professionals who need it to complete their portions of the adoption procedure. We strongly encourage you to be honest and forthcoming on the application, as most information is confirmed via thorough background checks.
